The Process for Providing Services

Any Age Medical Home Healthcare ensures that optimal care is provided with a step-by-step process. There is an intake done in the client's home to evaluate for services. The intake is always done by a qualified RN. The RN will review all necessary needs to provide a tailored care plan that fits the needs of the client. Clients with medical needs can rest easy knowing that our nurses will be in constant contact with physicians as needed. Our nurses and team members are trained to recognize when there is a change in a condition or situation requiring intervention. Pediatric Private Duty Care Nursing

We provide medical private duty nursing, or non-skilled hourly care that is comparable to the care received in the hospital in the comfort of your home from 4-24 hours per day. Our nurses are skilled in managing medical equipment and technology. They follow a consistent training regime at our agency to ensure high quality care.

We work with families to establish an optimal care plan based on the needs and requirements for the client to receive exceptional care.

These are a few of the situations that may qualify a medically fragile child for private duty nursing:

a child diagnosed with seizures.
a child that is dependent on oxygen
ventilator dependent
has a tracheostomy.
requires IV therapy.
requires nutrition from a gastrostomy tube (g-tube) or a nasogastric tube (ng-tube)
wound care.
Respite care.
Failure to thrive.

Pediatric Personal Care Services

At Any Age Medical Home Healthcare we understand the importance of family. We know that parents need help with their children that require specific attention at times. We are trained to care for pediatric patients that have special needs. Parents sometimes need assistance to be able to work, relax, or leave the home for errands. Our personal care assistants are extensively background checked because integrity and the safety of our clients are our top priority. We offer non-medical personal care assistance for pediatric clients that need help with:

bathing
grooming
mobility
light cleaning and food preparation
medication reminders
safety in the home
and a variety of other necessary activites of daily living

Adult Personal Assistance Services (help with ADL and IADL Assistance)

Our patient care assistants and caregivers will provide tailored care based on the client's specific needs. We assist the client with:

bathing and grooming
appointment reminders
self-medication reminders
help with ambulation
light housekeeping which includes changing bed linens, making beds, light dusting
taking out garbage
preparing meals and cleaning
picking up prescriptions
assistance with grocery shopping
help in answering the phone and or door
accompanying clients to lunch or dinner
